    Job Title | IT Program Manager (HRP and DSNP) Duration | 10 months Location | Remote Pay Rate Guidelines | $70/HR – W2 Duties | Spearhead end-to-end program management for DSNP and HRP initiatives, ensuring timely and budget conscious delivery of strategic objectivesDevelop and maintain detailed project roadmaps, timelines, RAID logs, and resource allocation plans to support ongoing operations and regulatory deadlinesKey liaison between cross-functional business teams (clinical operations, product, compliance) and IT delivery partners to align priorities and resolve dependenciesManage third-party vendors and system integrators, including Jiva platform stakeholders, to drive deliverables, uphold SLAs, and ensure seamless system performanceFacilitate governance processes including change control, requirements traceability, and post-deployment reviewsDeliver executive-ready communications: status reports, steering committee updates, and stakeholder engagement sessions to promote transparency and alignmentOversee system enhancements, integrations, and data flow optimizations to support DSNP/HRP functions – eligibility, assessments, care plans, and outreach workflowsPartner with compliance and regulatory teams to ensure IT systems met CMS and state-specific DSNP requirements, supporting audit preparation and documentation effortsLead system enhancement initiatives supporting compliance mandates such as Model of Care (MOC) tracking, risk stratification, and encounter data reportingLeverage tools like Jira and MS Project for robust project tracking, resource management, and cross-team visibilityProactively identify risks, integration issues, or scope deviations, collaborating across teams to develop mitigation strategies or escalate as neededReviewing and refining the project plansWorking cross functionally with IT, Claims, Regulatory, TeamsNot working directly with Vendors but will have input in their workReporting to Rahul – Associate Director of HRPUsing MS Project and working toward SmartSheetsStrong Excel Experience Top Requirements(Must haves) | HRP ImplementationsDSNP ImplementationsRegulatory ExperienceModel of Care (MOC)Regulatory AuditHealthCare ITJiva Implementation Experience What to look for in a project planHow do we do it?MS Project Additional Qualifications | Jiva Implementation ExperienceNice if they come from:KaiserCenteneBlue ShieldHealth NetLocal HealthPlans with Medicaid and Medical
