    Job Description

     

    A dynamic, fast-growing healthcare company is seeking a Member Outreach Specialist to work with the outreach team in increasing member population and being at the front line of patient advocacy. This is an ideal opportunity for a highly motivated professional who is detail-oriented and able to work efficiently, meet deadlines and be a key team contributor.

     

    JOB TITLE: Member Outreach Specialist       

     

    SUMMARY OF DUTIES: 

    Program introduction and enrollment of new patients to increase the market population. Identify patients who have not completed prevention health tests/screenings/visits and schedule them to complete those indicators. Educate and assist patients on the importance of prevention, health & wellness. Participate in health fairs and other community related projects. Identify and offer feedback on ways to improve the overall patient experience.

     

    ESSENTIAL FUNCTIONS INCLUDE BUT NOT LIMITED TO:

    • Perform duties to act as a liaison between the PCP office and the patient
    • Perform training and orientation within physician practices
    • Welcome and assist patients in different ways throughout various health centers
    • Explain program and benefits to member and enroll them via online application
    • Participate and assist with activities in our patient advocacy groups
    • Coordinate improved systems for patient education and completion of preventive services
    • Assist in making appointments and arranging referrals/consultations.
    • Conduct member outreach calls to increase market population
    • Assisting in any prevention and health & wellness campaigns
    • Assisting with patient satisfaction surveys
    • Conduct outreach to community resources on topics pertaining to prevention, health & wellness
    • Supports the Patient’s Bill of Rights/Responsibilities
    • Protects the Patient’s right to dignity, privacy, and confidentiality
    • Able to communicate effectively both verbally and in writing with other staff, patients, and providers
    • Works to ensure a safe and pleasant environment of care
    • Has the ability to maintain harmonious professional relationship with co-workers between all departments
    • Attends meetings according to departmental guidelines
    • Represents the organization in a positive and professional manner in the community
    • Complies with all organizational policies regarding ethical business practices
    • Anticipates needs and shows initiative
    • Demonstrates flexibility in meeting organizational needs

     

    QUALIFICATIONS/REQUIREMENTS:

    • High school diploma or equivalent
    • 2+ years of healthcare customer service experience
    • Bachelor’s degree in healthcare related field preferred
    • Must have good organizational skills and ability to handle multiple priorities and deadlines
    • Must be able to communicate effectively
    • Must be able to prioritize and cope with stress in a positive manner
    • Must be self-motivated and able to work independently and achieve results without supervision
    • Must be proficient in Microsoft Office Applications and web-based applications
    • Must be able to effectively present information and respond to questions from leadership
    • Adheres to all company policy
    • Bilingual Spanish/English preferred
    • Must always present a clean, professional appearance. Everyone is expected to be well-groomed and wear clean clothing, free of holes, tears, or other signs of wear
    • Must act in a professional manner in all dealings with patients, co-workers, and external customers

    CLASSIFICATION: Non-Exempt

    REPORTS TO: National Manager of Member Services

     

    SCHEDULE: Monday to Friday (office hours may vary, may include weekends)

     

    TRAVEL: 40% of the time, corporate office located in Jupiter, Florida

     

    ENVIRONMENTAL/WORKING CONDITIONS: Normal office environment.

     

    PHYSICAL DEMANDS: Requires walking, sitting, and standing associated with a normal office environment.
