    Overview Our growing home care agency seeks an In-Home Assessment & Care Plan Coordinator to join the team. You will work collaboratively with clients and their families to determine client care needs, preferences, and goals, by conducting comprehensive in-home assessments and consulting with physicians and other professional care providers.

    The Home Assessment & Care Plan Coordinator will also develop the Care Plan for our clients based on industry best practices.

    The ideal candidate is seeking a part-time time role with flexible hours.

    Essential Duties / Responsibilities

    1. Performs care assessments with prospective clients in their homes or by telephone; also performs re-assessments with current clients.
    2. Educates clients on agency services, programs, features, and benefits, and provides information about agency policies and procedures.
    3. Assures that the level of care established for clients is safe and appropriate based on client monitoring and in-home assessment.
    4. Serves as the primary point of contact for clients' care needs, addressing their inquiries and resolving issues promptly and professionally.
    5. Leads in-home orientations for new caregiver-client pairings.
    6. Coaches and trains caregivers to ensure consistent, high-quality care.
    7. Upholds confidentiality standards in accordance with HIPAA regulations.

    Qualifications ● High school diploma or equivalent

    ● Minimum of two years of experience performing in-home care assessments, care planning, implementation, and outcome evaluation

    ● A strong commitment to patient confidentiality and compliance with HIPAA regulations, as well as the ability to document assessment findings accurately and thoroughly

    ● Comfortable with closing/asking for business

    ● Proficiency in office software applications (e.g., Microsoft Office Suite)

    ● Must be comfortable entering personal living spaces while displaying a positive attitude

    ● Must have excellent verbal communication and follow-up skills ● LPN or RN experience strongly preferred
