Responsibilities

Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility.

Monitor and report changes in clients' conditions.

Provide companionship and ensure clients' comfort and safety.

Follow each client's individualized care plan.

Maintain accurate documentation of care provided.

Qualifications

Valid certification as a Certified Nursing Assistant (CNA) or equivalent licensure required.

Proven experience working with geriatric patients or individuals with disabilities in home healthcare or long-term care settings.

Ability to assist with ADLs efficiently while demonstrating empathy and patience.

Knowledge of home health care assistance regulations and best practices for client safety and confidentiality.

Physical ability to perform heavy lifting when necessary and use patient transfer methods safely.

Has a valid Driver's License and reliable transportation

Hoyer Lift experience is preferred

Schedule: Monday - Friday Mon / Wed / Fri: 5:00 PM – 8:00 PM (3 hrs/day) Tue / Thu: 5:00 PM – 9:00 PM (4 hrs/day)
