Excellent opportunity for a PsyD or PhD to join this innovative and established healthcare practice in Denver, CO.

This group cares for patients who reside in assisted living and personal residences, acting as both primary care provider and transitional care providers for patients who are homebound.

The group is seeking an exceptional Behavioral Health Care Manager who will be joining their behavioral team with a focus on geriatric patients with underlying psychiatric conditions.

• Initial assessments and treatment plans
• Consult with psychiatric consultants and modifications
• Tracking patient follow-up visits and progress
• Ongoing collaboration and coordination with treating providers
• Provision of brief interventions using evidence based treatments
• Relapse prevention planning and preparation for discharges
• Mostly virtual care management, but some live visits may be warranted
