    Licensed Behavioral Health Specialist opportunity in East Valley Phoenix suburb

    Excellent opportunity for a highly skilled and licensed Behavioral Health Specialist (LISAC, LASAC, LAC, LMSW, LCSW) to join our team in Mesa, AZ. As a Behavioral Health Specialist, you will be responsible for meeting with patients and counseling them through complex life situations. You will be assigned a territory in the East Valley and work with an integrated support team. All of the team is remote and in the field seeing 4 patients per day in the patient's home setting.

    This is a dedicated organization committed to taking care of the physical and social needs of high risk adult patients. Their approach is centered on leading with their hearts and providing customized care to each patient. This position allows the specialists to ask themselves, "How can I best help show this person care right now?"

    Responsibilities:

    • Foster relationships with patients by engaging with them in their home.
    • Help identify opportunities and work through past issues that may be causing conflict or stress in their lives.
    • Provide behavioral counseling and therapy as needed in a supportive and beneficial manner.
    • Be creative in helping patients reach their goals.
    • Work in a team environment that includes a NP, clinical liaison and coordinator to manage a panel of 40 patients
    • See an average of 4 patients per day
    • Approximately one hour meeting time allowed for each patient plus 30 minutes for admin. work and travel.
    • Assess patients' needs, develop treatment plans, and monitor progress
    • Collaborate with other team members to ensure comprehensive patient care
    • Maintain accurate and up-to-date patient records
    • Participate in regular team meetings and continuing education


    Requirements:

    • Valid and current Arizona State licensure as a LISAC, LASAC, LAC, LMSW, or LCSW
    • Ability to provide compassionate care to high-risk patients
    • Excellent communication and interpersonal skills
    • Ability to work in a team environment
    • Valid driver's license and reliable transportation
