Support individuals 18 and over with Case Management who have a mild or moderate developmental/intellectual disability as well as individuals who may be dual diagnosed. Some of the areas you will be assisting the Individuals with are money management; organizing and maintaining their household; personal safety; transport and advocate in medical appointments and other appointments; acquiring necessary benefits such as SSI, CalFresh, etc.; assist and provide reminders regarding personal health and hygiene; shopping; mobility training, if needed; socialization; and employment. Provide Independent Living Services (ILS), Supportive Living Services (SLS), Parent Training, and Integrated Employment Services.

Also instruct the Case Managers in providing the appropriate level of service for each individual served: e.g., using your creativity for an individual who may exhibit resistance or opposition to an action or idea; raising the integrity of the case management services provided to an individual; ability to supplement the knowledge the Case Managers may bring to their caseload; provide a different perspective to difficult cases; work as a team member with the Agency Administrator and Director to give the population we serve the best possible experience.
