    Position: Information & Referral Specialist Rev. 7/26

    Reports to: Community Services and Information Supervisor

    Basic Function: To provide referral assistance, information, outreach services, as well as

    maintain up-to-date comprehensive resource files.

    Responsibilities:

    • Triage Calls for referrals, information, and Case Manager on Call Support.
    • Enter referrals that are received into the computer system.
    • Complete documentation for Caremanager On Call assistance that you provided.
    • Assist with booking transportation for consumers with the contracted providers that Tri-Valley utilizes.
    • Monitor the Email Boxes, Voice Mail Box, and Information Box and provide necessary follow-up work.
    • Maintain and update Resource Library on community agencies, benefits, and other informational resources that are often requested.
    • Encourage utilization of resource materials; serve as a staff and public contact on available resource information.
    • Provide training and support for staff regarding I&R services including participating with the I&R Team in creating a monthly newsletter for staff
    • Gather statistical data monthly.
    • Examine statistics and make recommendations about where outreach activities should be focused.
    • Complete presentations and outreach to increase knowledge in the community of the function of I&R and the services that Tri-Valley, Inc. offers.
    • Participate in a minimum of 40 hours a year of training around I&R related topics.
    • Maintain and update Emergency Disaster Resources and Emergency Shelter Resources.
    • Participate in staff and team meetings to discuss homecare and other related issues.
    • Maintain work area in a neat and professional manner.
    • Perform miscellaneous duties and assignments as directed by immediate supervisor.
    • Meet with supervisor on regular basis.

    Essential Functions/Qualifications:

    High School Diploma required; College Degree preferred; minimum of 2 years of relevant experience in a social services environment.

    Physical Demands:
