AleraCare has an opening for a p art-time Infusion Nurse Practitioner at our Sherman Oaks, CA infusion center. We are looking for coverage 2-3 days per week between Mon-Fri 8am-5pm. AleraCare is a provider of infusion treatments and specialty pharmacy services for high need and medically complex populations. We have 35 infusion centers in 6 western states and we're growing!
Description of Responsibilities
The Nurse Practitioner is responsible for providing nursing services to assigned patients in accordance with the patient’s plan of care.
Responsibilities:
- Maintains patient confidentiality at all times.
- Functions as a patient advocate; treating patients and families with respect and dignity.
- Demonstrates competency in providing patient care and provides care in accordance with professional standards and the state’s nurse practice act.
- Performs initial patient evaluations/assessments and develops a written plan of care.
- Prepares clinical and progress notes, coordinates services, and informs the physician and other personnel of changes in the patient's condition and needs. Responsible for the clinical record of each patient receiving nursing care. Ensures documentation meets all standards and policies, including documentation of all findings, interventions, and results.
- Assures that progress reports are succinct when the patient’s condition changes or deviates from the plan of care.
- Regularly reassess the patient’s condition, nursing needs, and update the plan of care.
- Furnishes those services requiring substantial and specialized nursing skills.
- Initiates appropriate preventive nursing procedures.
- Collaborates with physicians and other members of the healthcare team with optimal care in mind for the patient.
- Evaluates and reevaluates the patient and/or family for changing need for services. Provides patients and/or family with health education, counseling, and referrals to other healthcare professionals and/or community resources when appropriate.
- Orders and/or interprets lab work and diagnostic tests according to patient need and/or plan.
- Demonstrates the ability to prescribe medications and non-pharmacological therapies based on the patient’s diagnoses, health history, and age.
- Initiates case conferences and communicates the patient plan of care with team members.
- Participates in the company’s Performance Improvement and Continuous Quality Improvement (CQI) programs.
- Participates in committees when requested.
- Attends continuing education programs and staff meetings as appropriate to job responsibilities.
- Assumes responsibility/ownership of clinic location(s).
- Supports and maintains a culture of safety and quality care.
- Stays current with medical advancements, new technology, new drugs through inservices, continuing education classes, society meetings, professional journals, etc.
- Performs other duties as assigned by supervisors.

Minimum Qualifications
- Master’s Degree from an accredited Nurse Practitioner Program.
- Holds a current NP state license in state of practice.
- At least 4-5 years of advanced practice registered nursing (APRN) experience, ambulatory and infusion nursing experience is preferred.
- Strong interpersonal, time management, and organizational skills.
- Strong written and verbal communication skills.
- Experience with electronic medical records (EMR). Must have organizational skills to maintain and manage accurate patient care records.
- Ability to recognize and direct information to the appropriate health care provider.
- Computer skills that include word processing and efficient use of the internet and e-mail.
- Current CPR card, advanced practice certification, current APRN licensure, valid driver’s license, reliable transportation, automobile insurance.
