Position Summary
The Infusion Nurse is a mobile registered nurse who delivers on-site IV infusion therapy, blood transfusions, and type and screen draws directly at skilled nursing facilities (SNFs) and other designated client sites across an assigned regional pod. You will drive to client locations daily, set up and administer treatments, monitor patients during and after procedures, document care, and move to the next facility. This is a fully field-based role.
Mobile Clinical Delivery
Provide infusion services at assigned facilities within your region daily
Coordinate with facility staff (DON, charge nurse, or designee) upon arrival
Review patient orders and set up infusion workspace
Cover multiple facilities per day based on caseload and geography
IV Infusion Administration
Perform peripheral IV infusions per patient orders and RN scope of practice
Assess patients pre-infusion; monitor vitals throughout
Manage infusion pumps and supplies
Document all clinical observations in real time
Blood Transfusions (If Assigned and Training Requirements Completed)
The following responsibilities apply only to nurses who have completed Vellum's required transfusion training, demonstrated competency, and been formally assigned to the transfusion team by the Company.
Administer blood product transfusions on-site at SNFs
Complete pre-transfusion two-nurse verification where required
Monitor for adverse reactions throughout; intervene per Vellum transfusion reaction protocol
Complete all post-transfusion documentation per Vellum protocol
Type & Screen Collection
Perform venipuncture for type and screen specimens
Label, handle, and coordinate specimen transport per Vellum and facility protocols
Patient Assessment & Monitoring
Conduct nursing assessments before, during, and after all procedures
Monitor for adverse reactions, infiltration, and other complications
Activate emergency response when indicated; manage patient continuously until EMS assumes care and a full verbal handoff has been completed
Documentation
Complete all clinical documentation in Vellum's app during or immediately after each encounter
Meet Vellum's internal standards and payer-specific requirements to support accurate billing
Complete all charts same day; all documentation must be finalized within 24 hours of service delivery
Facility Coordination
Communicate with SNF staff on patient status, scheduling, and supply needs
Build positive working relationships to support client retention
Supply & Equipment Management
Maintain mobile supply kit per Vellum's supply management policy
Stock IV supplies, blood administration sets, specimen tubes, dressings, and emergency equipment
Submit resupply requests proactively — before a shortage occurs
Pod Lead Responsibilities (When Assigned)
Experienced nurses may be designated Pod Lead, performing all standard duties plus:
Schedule clinic days and coordinate logistics across pod territory
Coordinate PRN nurse scheduling to ensure full coverage
Orient and train new hires, including ride-alongs and competency validation
Report regularly to Regional Clinical Manager on performance and staffing
Conduct site visits for newly onboarded client facilities
Partner with Account Manager on client engagement and clinical inquiries
Qualifications
Required
Active RN license in the state of practice; compact license accepted where applicable
2+ years of clinical nursing experience with direct IV therapy and/or infusion administration
Competency in peripheral IV insertion, central line access and maintenance, blood product administration, and venipuncture
Current Basic Life Support (BLS) certification
Strong independent clinical judgment
Excellent documentation skills with ability to complete thorough, accurate charts in a mobile environment
Valid driver's license, clean driving record, and reliable personal vehicle suitable for daily regional travel
Current auto insurance meeting Vellum's minimum coverage requirements
Physical ability to lift and carry supply bags (up to 30 lbs), stand for extended periods, and perform clinical procedures in varied facility environments
Preferred
CRNI (Certified Registered Nurse Infusion) or VA-BC (Vascular Access Board Certified) certification
Experience with blood product transfusion in non-hospital settings (SNF, home health, ambulatory)
Prior experience in home infusion, mobile health, or SNF-based clinical services
Experience with electronic scheduling/dispatch systems and mobile clinical documentation
Knowledge of Medicare/Medicaid documentation requirements for infusion services
