Registered Nurse Contract Home Care

Responsibilities:

• Completes initial comprehensive and ongoing assessment of patient and family to determine needs
• Admissions of patient for services and development of the initial patient/family Plan of Care
• Uses health assessment data to determine nursing diagnosis
• Coordination of services with the patient, family, and interdisciplinary team members
• Ongoing clinical supervision of Licensed Practical Nurses and Home Health Aids
• Perform skilled care as needed as treatment RN

Requirements:

• Valid Registered Nurse (RN) license
• RN Diploma, Associates or B.S. degree in nursing
• Minimum 1 year of clinical experience, home care preferred
• Valid Driver’s License, auto insurance and reliable transportation
• Excellent observation, problem solving and communication skills
• Compassion and integrity
