    Advanced Practice Pharmacist Neurology and CID

    Position Overview:

    Will initiate, modify and discontinue medications within a Collaborative Practice Agreement as defined by the California State Board of Pharmacy. Focus on management of complex disease states to help patients reach optimal therapeutic goals. Simplify patient medication regimens, minimize the risk of harmful medication interactions and educate patients to enhance adherence and clinical outcomes. Responsible for policy development, Continuous process improvement, continuing education, clinical and medical staff development and Pharmacy APPE student and resident preceptorship. Performs patient care assessments, orders and interprets laboratory tests, prescribes, modifies and discontinues medication therapies. Monitors clinical outcomes, communicates with medical providers, performs electronic documentation and refers patients to other health care providers.

    Preferred Criteria:
    \- Experience as a specialty pharmacist in in a hospital setting.
    \- Board Certification (ex: BCACP/BCPS)
    \- EPIC and Beacon experience.

    Job Summary/Responsibilities:

    \- Proficient computer experience and accurate data entry skills in Word and Excel.

     

    Job Description:

    These Principal Accountabilities, Requirements and Qualifications are not exhaustive, but are merely the most descriptive of the current job.     JOB ACCOUNTABILITIES:

    PATIENT CARE.
    •Meets with patients individually to provide medication support and monitoring either by 1)managing directly or 2)in partnership with physicians, advanced practice nurses, physician assistants, dieticians, medical assistants, care coordinators, and other members of the care team.
    •Reviews medication profile to ensure safety and proper use of medications. Alert to actual or potential medication affordability/adherence issues, interactions, therapeutic duplications, medication allergies and potential medication errors. Triages and solves medication related problems demonstrating initiative and sound judgement.
    •Evaluates and documents that the medication(s) selected is appropriate for achieving the desired therapeutic outcome. Communicates potential problems and likely solutions to the prescriber.
    •Adjusts therapy as indicated by patient condition and laboratory results.
    •Educates patients and their families about medications.


    COMMUNCATION AND EDUCATION.
    •Utilizes and communicates the appropriate prescribing of medications consistent with national clinical guidelines, evidence-based medicine, and best practices.
    •Communicates professional knowledge effectively in both oral and written forms to patients and other health professionals.
    •Develops communication materials to support clinical initiatives, as needed.
    •Presents clinical information to patient groups, as needed.
    •Participates in Medical Resident, Pharmacy Resident, and Pharmacy Student Training.


    DOCUMENTATION.
    •Maintains a comprehensive EPIC patient profile. The profile will include pertinent information which may influence medication therapy decisions.
    •Maintains a current medication list in EPIC by performing medication reconciliation at each patient visit.
    •Documents all patient activity in the EPIC system. Collects data, maintains logs, records and other documentation as required.


    QUALITY AND COMPLIANCE.
    •Participates in quality improvement and medication use review activities.
    •Organizes, prioritizes, and plans workload within given time constraints. Works cooperatively with other members of the team to assure established objectives are accomplished.
    •Ensures compliance with hazardous waste collection and disposal procedures.
    •Maintains proficiency and expertise in area(s) of practice through continuous professional development.
    •Develops and maintains relationships with all customers.

    EDUCATION:

    Other: Graduate of an accredited school of pharmacy in the U.S

    Completion of PGY1 and/or PGY2 pharmacy residency OR 3+ years of clinical Neurology and Chronic Inflammatory diseases states experience.

    TYPICAL EXPERIENCE:

    5+ years recent relevant experience

    Meet any of two (2) of the following requirements:
    1\. Have certification from an organization recognized by the Accreditation Council for Pharmacy Education or another entity recognized by the California State Board of Pharmacy.
    2\. Have completed a postgraduate residency through an accredited postgraduate institution where at least fifty percent (50%) of the experience included the provision of direct patient care services with interdisciplinary teams.
    3\. Have provided clinical services to patients for at least one (1) year under a collaborative practice agreement or protocol with a physician, advanced practice pharmacist, pharmacist practicing collaborative drug therapy management, or health system.

    CERTIFICATION & LICENSURE:

    PHARMR-Registered Pharmacist Upon Hire from CA Board of Pharmacy

    ASHP-Post Graduate Year 1 ASHP General Pharmacy Residency Upon Hire

    Hold an active APh license in good standing with CA Board of Pharmacy or sufficient experience and certification/s to become eligible for APh license.

    SKILLS AND KNOWLEDGE:

    Must possess superior written and verbal communication skills. Must demonstrate appropriate clinical patient management skills, initiative, excellent judgement, exceptional interpersonal skills, and the ability to multitask and maintain composure.

    Must be self-motivated and able to quickly adapt to meet a variety of clinical and operational needs.

    Professional demeanor and interactions with all colleagues and patients.

    Interpretation and understanding of patient specific medication orders.

    Proficient at Pharmacy order entry, verification of CPOE, and familiar with RN and MD charting within EMR.

    Ability to maintain concentration and accuracy despite a busy environments with distractions and medications with a narrow therapeutic window and may potentially cause harm if inappropriately prescribed, prepared, or administered.

    Comfort working in a stressful and high acuity patient care setting.

    Exceptional communication with all healthcare providers.

    Comfort and skill interviewing patients, family members, medical providers, and pharmacy staff in an effort to facilitate medication reconciliation.

    Collaborations and oversight of other pharmacy staff in order to maintain an environment of medication safety.

    Accuracy, comfort and skill managing chemotherapy plan templates within an EMR

    Serve as a pharmacy and medication management resource to oncology medical and clinical staff.

    Collaboration with oncology leadership, medical staff, and pharmacy leadership to maintain and facilitate financial stewardship and clinical excellence/care.

    Serve as a pharmacy representative for regulatory compliance surveys for inquiries dealing specifically with the verification, preparation, dispensing, storage, waste, and administration of chemotherapeutic or hazardous medications.
