About the Role:

Responsibilities:
Conduct comprehensive assessments of patients' needs during after hours.
Coordinate with healthcare providers to ensure seamless patient care.
Manage and prioritize case loads effectively in a fast-paced environment.
Provide support and guidance to patients and their families.
Document all case management activities accurately and timely.
Collaborate with multidisciplinary teams to optimize patient outcomes.
Stay updated on policies and procedures relevant to inpatient care.
Requirements:
Active RN license in the state of California.
Minimum of 2 years of case management experience in a healthcare setting.
Strong communication and interpersonal skills.
Ability to work independently and make critical decisions.
Experience with electronic medical records (EMR) systems.
Knowledge of healthcare regulations and compliance standards.
