    About the Role:

    Join MedPOINT Management as an Inpatient Transition of Care Coordinator and play a vital role in enhancing patient outcomes. In this position, you will facilitate seamless transitions for patients from hospital to home, ensuring they receive the care they need. Be part of a dedicated team committed to making a difference in the lives of our patients in Sherman Oaks, CA. This is a full-time, temporary position.

    Responsibilities:

    Coordinate post-discharge care plans and follow-up appointments for patients.

    Conduct comprehensive assessments to identify patient needs and barriers to care.

    Collaborate with healthcare providers to ensure continuity of care and medication management.

    Educate patients and families on discharge instructions and available resources.

    Monitor patient progress and adjust care plans as necessary to promote recovery.

    Document all interactions and care plans in the electronic health record (EHR) system.

    Participate in quality improvement initiatives to enhance patient care processes.

    Act as a liaison between patients, families, and healthcare teams to advocate for patient needs.

    Requirements:

    Bachelor's degree in Nursing, Social Work, or a related field preferred.

    Current RN or LCSW license in California is required.

    Minimum of 2 years of experience in care coordination or case management.

    Strong understanding of healthcare systems and community resources.

    Excellent communication and interpersonal skills for patient interaction.

    Proficient in electronic health record (EHR) systems and documentation.

    Ability to work independently and manage multiple priorities effectively.
