About the Role:

Join MedPOINT Management as an Inpatient UM Nurse Case Manager in Sherman Oaks, CA, where you will play a vital role in patient care coordination. Be part of a dynamic team dedicated to improving health outcomes and ensuring high-quality care for our patients.

Responsibilities:

Conduct comprehensive assessments of patients to determine care needs.

Collaborate with healthcare providers to develop and implement care plans.

Monitor patient progress and adjust care plans as necessary.

Facilitate communication between patients, families, and healthcare teams.

Ensure compliance with regulatory and accreditation standards.

Educate patients and families about treatment options and resources.

Document all case management activities in accordance with policies.

Participate in quality improvement initiatives and team meetings.

Requirements:

Active RN license in California with a minimum of 2 years nursing experience.

Experience in case management or utilization management preferred.

Strong clinical assessment and critical thinking skills.

Excellent communication and interpersonal skills.

Ability to work collaboratively in a fast-paced environment.

Knowledge of healthcare regulations and managed care principles.

Certification in Case Management (CCM) is a plus.

Proficient in electronic health records and documentation practices.
