    JOB SUMMARY

    Works in close collaboration with professional nursing staff to promote and ensure that quality care is provided, and outcomes are achieved in a timely and cost-effective manner. Utilizes an interdisciplinary process to assess, plan and provide patient care for all patients in Resilience Healthcare. Responsible for meeting the established Resilience Healthcare Standards of Nursing Practice in the management of all assigned patients. Assists physicians with the development and implementation of wound treatment plans and participates in performance improvement review activities. Acts within the guidelines of federal and state laws, JCAHO Standards and professional and ethical practice.

    JOB QUALIFICATIONS

    1. Registered Nurse with current Illinois license required

    2. BSN preferred

    3. Two years of Wound Care experience.

    4. CPR certification.

    5. Wound Ostomy Certification Required

    Pay Rate: $35 - $50 per hour

    JOB DUTIES

    1. Demonstrates the Resilience Healthcare Customer Service Values, which are key in providing quality service to patients and customers. *

    2. Assesses health status, functional patterns, medical needs, reassessments for discharges and rehabilitation potential on high risk/problem prone on all admissions to Resilience Healthcare Chicago.

    3. Identifies real and potential problems requiring interventions prior to moving the patient to the next level of care.

    4. Educates the patient in an age-appropriate manner to care for alternatives and outcomes and facilitates the implementation process.

    5. Assumes responsibility for the maintenance of effective skin care, the prevention and treatment of wounds, and functions in a consultative role to physicians and nurses.

    6. Monitors and evaluates progress of patients towards expected outcomes in relation to goals by gathering data from pertinent sources.

    7. Utilizes hands on physical assessment skills and interview techniques to formulate a clinical database; analyzes collected information derived from patient and other sources to identify age-appropriate patient problems and needs. Re-assesses on an ongoing basis and documents findings or changes in condition in the progress notes. Assesses severity of findings and obtains physician intervention as warranted.

    8. Uses data gained through assessment and collaboration with the health care team to develop a plan of care for each identified patient problem. Identifies patient care situations that require intervention, implements nursing actions and evaluates responses.

    9. Ensure desired outcomes that are mutually agreed upon by the patient, family (when appropriate) and the nurse.

    10. Formulates desired outcomes that are specific and measurable within a certain time frame and consistent with other heath provider’s expectations.

    11. Assign/delegates care to other qualified members of the health care team. Coordinates care between other professional disciplines as needed.

    12. Documents select data on appropriate forms and enter designated information into the computer. Actively participates in the Quality Assurance and Improvement Process.

    13. Accurately completes the superbill and ensures that all orders and charges are initiated accurately.

    14. Implements wound care and HBO protocols when clinically necessary.

    15. Demonstrates responsibility for own professional practice and growth by ensuring clinical knowledge base and skills are maintained and enhanced.

    16. Organizes on a continuum due to the changing status of the patients and environment. *

    17. Supports department of nursing and hospital policy and procedures. Complies with department policy and procedures. Comply with staffing rotations.

    18. Maintains environment and equipment in an appropriate manner. Supports unit teams. Resolves problems. Supports customer service.
