    Insero Talent Solutions is recruiting a Nurse Manager for an exclusive senior living community in Rochester, NY.

    Position Summary:

    The nurse manager is responsible for the supervision and management of the nursing department, the organization and administration of the clinical, physical, and emotional needs of each resident within the guidelines of NYSDOH Assisted Living/Enriched Regulations. The nurse manager will work closely with the interdisciplinary team to ensure the program goals to optimize function, health; safety and overall well-being are maintained during care provision, working to create a person centered environment.

    Essential Job Functions:

    • Provides supervisory oversight to ensure quality of care is delivered through regular daily reviews of physician’s orders, required documentation, and chart audits. Ensure daily system checks are maintained and documentation is accurate per policy and according to the NYS Department of Health regulations.
    • Monitor, coach and teach staff regarding appropriate approach with residents and address issues as they occur.
    • Assist with staff schedule and payroll documentation.
    • Ensure staffing coverage at all times.
    • May also be responsible for providing on-floor staff coverage on weekends, evenings and nights as required and medication pass when staffing levels are not optimal.
    • Participate in meetings with Department Managers, and any other agencies providing services to the resident to maintain the quality of services provided.
    • Coordinate with Case Manager and Director of Assisted living to implement completion of monthly in-services programs for the resident care staff and additional training as required.
    • Completion of 90-day and annual staff performance appraisals, to include employee performance plans when required.
    • Assist with continued development of new programs, policies and procedures, training curriculum and/or committee development.
    • Participate in the hiring process, interviewing prospective team members as required to maintain an appropriate staffing module.
    • Review Individual Service Plan (ISP) upon admission including creation of Medication Administration Records (MARS) and flow charts. Reviews of ISPs on a regular basis with updates to MARS and flow charts as needed.
    • Develop and foster positive relationships with family members, keeping families connected and involved with their family member’s service plan.
    • Work with all departments to identify resident or family concerns to determine resolutions and implement departmental changes as required. Complete care notes as required documenting concerns and resolutions as they pertain to the resident care department.
    • Ensures all residents are medically recertified and appropriate to reside at the assisted living level of care through completion of annual NYSDOH required paperwork.
    • Work with case manager to arrange transfer to another care level if needed.
    • Keep the physician and families updated on changes in the resident status and ensure the physician instructions regarding care are met.
    • Assist case manager in assessing the functional level of applicant as needed.
    • Review all medical information.
    • Ensure all DSS 3122s are completed per regulation and accurately.
    • Recommend admission status for potential applicants as requested by case manager.
    • Work with residents’ primary physicians to coordinate care needs and medical orders.
    • Maintain all resident nursing records as required.
    • With the physician, assess residents’ capability of self-medication and review, periodically after admission, the residents’ continuing ability to self-medicate.
    • Supervise resident care assistants to ensure medication management per MD order.
    • Assume responsibility for proper storage of all medications not kept by the residents.
    • Maintain electronic health record & medical records, related to medication orders including new prescriptions and medication changes, and ensure all medication orders are signed.
    • Ensure that nursing staff follow eMAR instructions and that the eMAR in accordance with department of health regulations.
    • Keep nurses and case manager updated on medication changes when needed.
    • Phone in refills for residents on med-assist program and notify resident’s family if they pick up refills.
    • Provide resident teaching re: medication and medical equipment as needed.
    • Update policies and procedures regarding nursing program notifying appropriate staff as needed.
    • Ensure all nursing staff follow policies and procedures.
    • Partner with the Director of Assisted Living to establish quality assurance systems to monitor all care and services for residents of the home.
    • Conduct frequent audits regarding nursing program as needed and present them to the management team.
    • Communicate all pertinent information to other staff
    • Ensures that resident rights are maintained and confidentiality is upheld.
    • Attend and participate in monthly Family Council Meetings and Assisted Living Resident Council Meetings.
    • Assist Director of Assisted Living and Case Manager with admissions or readmissions that are medically complex.
    • Attend workshops, seminars, conferences for professional development and in-services as required.
    • Complete any additional duties as required by Assisted Living Director
    • Shares in on-call rotation
    • Serves as back-up to the Case Manager

     

    Qualifications:

    A combination of experience, education, and/or training which substantially demonstrates the following knowledge and skills:

     

    • Graduation from an approved RN school of nursing, current New York State License.
    • At least five years’ experience working with an adult population providing supportive care services in an adult care facility or long term care building. Three years of supervisory experience required.

    • Ensures that resident rights are maintained and confidentiality is upheld under the Health Insurance Portability and Accountability Act (HIPAA).
    • Attend workshops, seminars, conferences for professional development and in-services as required.
