    JOB SUMMARY

    Inspire Hospice and Palliative Care continues to grow in the metro Atlanta area and is looking for a PRN Hospice RN for the Gwinnett and surrounding counties area. The Hospice RN works with patients, their families, and other health care professionals to provide and coordinate high-quality end of life care. Hospice RNs work in private homes, assisted living facilities, and skilled nursing facilities. They provide education to patients and families and work to ensure patients are comfortable in their home environment. They supervise licensed practical nurses and nursing aides.

    QUALIFICATIONS:

    • CURRENT GA REGISTERED NURSE (RN) LICENSURE REQUIRED.
    • Must possess a valid state driver's license, automobile liability insurance, and have dependable transportation
    • Minimum of one year of clinical experience is strongly preferred
    • Previous home health or hospice experience is preferred, but not required.
    • Must be capable of compassionate communication with patients and families, have keen observation skills and knowledge of when to alert doctors and others about changes in patients' conditions

    RESPONSIBILITIES:

    • Completes comprehensive assessments of patients and families and educates on hospice care. Provides a complete physical assessment and history of current and previous illnesses.
    • Provides professional nursing care by utilizing all elements of nursing process.
    • Initiates the plan of care and makes necessary revisions as patient status and needs change.
    • Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
    • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care.
    • Provides education to the patient as appropriate per assessment and plan.
    • Ensures that arrangements for equipment and other necessary items and services are available.
    • Supervises ancillary personnel and delegates responsibilities when required.
