About the Role:

Join HomeWell Care Services as an Intake & Care Relationship Coordinator, where you'll play a crucial role in connecting clients with our exceptional home care services. This position is perfect for individuals who are passionate about enhancing the lives of others and ensuring seamless care coordination.

Responsibilities:

Conduct initial client intake assessments to understand care needs and preferences.

Coordinate care plans in collaboration with clients, families, and healthcare professionals.

Build and maintain strong relationships with clients and their families to ensure satisfaction.

Respond to inquiries and provide information about services and resources.

Assist in the onboarding of new caregivers and ensure proper training.

Monitor ongoing client care and address any concerns or changes in needs.

Maintain accurate documentation and records of client interactions and care plans.

Participate in team meetings and contribute to continuous improvement initiatives.

Requirements:

Proven experience in care coordination or a related field, preferably in home care.

Strong communication and interpersonal skills to engage with diverse clients.

Ability to assess client needs and develop tailored care plans.

Proficient in using electronic health record (EHR) systems and documentation.

Compassionate demeanor and a genuine desire to help others.

Valid driver’s license and reliable transportation for client visits.

Knowledge of healthcare regulations and best practices in home care.

High school diploma or equivalent; relevant certifications are a plus.
