The Intake Coordinator is responsible for facilitating the onboarding of new referrals and processes for home health patients. This position is Full-time and hourly, and the individual will work with the rest of the teams, as well as being able to work independently as a contributor to the home health care team. The Intake Coordinator will play a crucial role in providing compassionate onboarding, verifying insurance, and supporting documents to patients on their journey back home from inpatient facilities. Responsibilities: • Verifying medical insurance coverage and benefits for each patient • Create EMR records • Answering phone and email inquiries • Verifying if we can accept patients from facilities • Performing clerical work, such as faxing, downloading, and scanning • Assigning Nursing staff to patients who need an initial start-up visit • Complying with the regulations and policies of the facility • Gather all data needed from facilities to meet the medical needs of patients • Obtain prior authorizations and verifications Qualifications: • High school diploma or GED • Strong skills in customer service • Working knowledge of EMR systems • Excellent organizational skills • Great communication skills • Proficient in computer literacy • Ability to work with hospitals and skilled nursing facilities • Conduct insurance verification • Obtain medical records from facilities • Review documents for clinical staff • Prepare new patient information Compensation: $15 - $17 hourly
