    Pay: $23/hour Classification: Exempt Schedule: Part-time (with strong potential to become full-time) Reports To: Operations Manager or Administrator

    Immediate need. Same-week start available. HomeWell Care Services is actively seeking an experienced Intake Coordinator who can step in quickly and support our growing referral volume. This role is essential to ensuring clients and families receive timely, compassionate access to care.

    Key Responsibilities

    Receive and process referrals from hospitals, physicians, and families

    Conduct intake calls and initial phone screenings

    Assess client needs and eligibility for HomeWell services

    Verify demographics, insurance benefits, and authorizations

    Accurately document all intake information in WellSky and agency systems

    Coordinate with sales, care managers, and clinical teams for timely admissions

    Serve as a professional and compassionate point of contact for clients and families

    Notify the Care Manager of accepted clients and ensure smooth care transitions

    Qualifications

    1–2 years of experience in healthcare, home health, hospice, or intake coordination

    Direct home health or hospice intake experience strongly preferred

    Excellent communication, organization, and customer service skills

    Strong attention to detail and problem-solving ability

    Proficiency in Microsoft Word, Excel, and WellSky EHR

    Knowledge of insurance verification, medical terminology, and HIPAA compliance

    Education:

    High school diploma or equivalent required

    Associate or bachelor’s degree in a related healthcare field preferred
