    Job Overview: As an Intake Coordinator at HomeWell Care Services, you will be responsible for managing the intake process, from receiving and responding to inquiries to coordinating patient assessments and facilitating smooth transitions into care. This role requires excellent communication, organization, and customer service skills, as well as a strong understanding of healthcare processes, insurance verification, and client coordination.

    Key Responsibilities:

    Complete Client/Patient Initial Inquiry: Receive and respond to inquiries via phone and email from referral sources (hospitals, doctors, social workers) and potential clients, gathering necessary information, explaining services, and determining if the agency can meet their needs.

    Information Gathering: Collect detailed client information including demographics, medical history, clinical needs, social history, emergency contacts, and insurance details to ensure appropriate care planning.

    Insurance Verification & Authorization: Determine potential payer sources, verify insurance benefits and eligibility, obtain pre-authorizations for services, and negotiate rates if needed.

    Referral Coordination: Coordinate referrals from various sources, ensuring all necessary documentation is collected and shared with the clinical team, and communicate referral status to relevant parties.

    Scheduling and Admissions: Schedule appointments for initial in-home assessments and assist with the creation of patient records, ensuring the data is entered accurately into the scheduling system.

    Documentation and Record Keeping: Maintain accurate and up-to-date client records in compliance with regulatory requirements (e.g., HIPAA, CMS), ensuring all medical, intake, and referral documentation is complete.

    Patient Education and Support: Answer client and family questions about services, policies, and procedures, addressing any concerns or complaints that may arise.

    Care Coordination: Collaborate with the clinical team and other departments to ensure a smooth transition of clients into services and ensure continuity of care.

    Required Qualifications:

    Education: High school diploma or equivalent required; Associate’s or Bachelor’s degree in healthcare administration or a related field preferred.

    Experience: At least three years of healthcare or home care experience, preferably in patient intake or coordination.

    Skills & Knowledge:

    Strong knowledge of medical terminology, insurance processes, and healthcare procedures.

    Excellent verbal and written communication skills for interacting with clients, families, and healthcare professionals.

    Ability to actively listen and gather complete, accurate information.

    Exceptional organizational and time management skills to handle multiple referrals and schedules.

    Proficiency with Electronic Health Record (EHR) systems and standard office software (e.g., Microsoft Office).

    Understanding of HIPAA and CMS regulations and compliance standards.

    Empathy, strong customer service focus, and a client-centric approach.

    Problem-solving skills for addressing client concerns and unexpected challenges efficiently.

    Preferred Qualifications:

    Experience with home health software such as HomeCare HomeBase.

    Previous experience in a healthcare or home care setting, particularly in intake coordination.

    Nursing license (LPN) or Certified Nursing Assistant (CNA) certification is a plus.
