    Status: Full-Time in office

    We are currently seeking an experienced, detail-oriented Intake Coordinator to support our Phoenix, Arizona home health operations.     As an Intake Coordinator, you will be one of the first points of contact in a patient’s home health journey. Your coordination, attention to detail, and communication skills will help ensure patients receive the care, services, and support they need — when they need it.

    You will serve as a key connection between hospitals, physicians, families, insurance providers, and our clinical care team.

    What You’ll Do

    • Manage the intake process from start to finish

    • Collect and organize patient medical, insurance, and personal information

    • Coordinate with hospitals, clinics, physicians, and referral sources

    • Help ensure smooth transitions into home health care

    • Assess patient needs and match them with appropriate services, treatments, and equipment

    • Verify insurance coverage and assist with authorizations

    • Maintain accurate electronic medical records and ensure documentation is complete and compliant

    • Serve as a compassionate resource for patients and families throughout the intake process

    What We’re Looking For

    • Required: Previous home health experience

    • Strong understanding of medical terminology and EMR systems

    • Experience with WellSky/Kinnser preferred

    • Excellent attention to detail and organizational skills

    • Strong communication skills with patients, families, providers, and internal teams

    • Ability to work independently while contributing to a supportive team environment

    • Flexible, adaptable, and comfortable in a fast-paced setting

    Preferred Qualifications

    • Associate’s or Bachelor’s degree in a healthcare-related field

    • Experience with insurance verification and authorizations

    • Bilingual skills are a plus
