Due to increased patient volume, Duval Vascular Center is seeking a dedicated and organized Intake Coordinator PRN to join our dynamic team. The Intake Coordinator will be responsible for managing the initial intake process for clients, providing essential support to ensure a seamless experience while collecting accurate information and assessing their needs. This role requires excellent communication skills, attention to detail, and the ability to handle sensitive information with professionalism and confidentiality.
Key Responsibilities:
1. Client Interaction:
- Serve as the first point of contact for prospective clients, answering inquiries and providing information about services offered.
- Conduct initial assessments to understand the needs and circumstances of clients.
2. Data Collection and Management:
- Collect and maintain accurate and confidential client data, ensuring all information is updated in the database.
- Complete intake forms and documentation in compliance with company policies and regulations.
3. Coordination of Services:
- Collaborate with internal teams to coordinate referrals and services as needed.
- Schedule appointments and follow-up meetings for clients with appropriate staff members.
4. Documentation and Reporting:
- Maintain organized and up-to-date records of client interactions, assessments, and referrals.
- Assist in generating reports and compiling data for program evaluation and improvement.
5. Communication:
- Communicate effectively with clients, caregivers, and team members to provide updates and address concerns.
- Provide education and information to clients regarding available resources and support services.
6. Continuous Improvement:
- Identify and recommend process improvements for the intake procedures to enhance efficiency and client satisfaction.
- Stay informed about company policies, procedures, and industry trends.
Qualifications:
- Bachelor’s or AA degree in social work, healthcare administration, or a related field preferred.
- Prior experience in client intake or administrative roles within a hospital or specialty clinic setting is highly desirable.
- Excellent interpersonal and communication skills, both verbal and written.
- Strong organizational skills and attention to detail.
- Proficiency in Microsoft Office Suite and experience with client management software.
- Ability to handle sensitive information with discretion and maintain confidentiality.
- Compassionate and empathetic demeanor, with a strong client-focused approach.
