Evolve Yourself LLC is seeking a compassionate, organized, and professional Intake Coordinator to serve as the first point of contact for clients seeking mental health services. The Intake Coordinator is responsible for managing referrals, conducting intake screenings, scheduling appointments, verifying insurance eligibility, maintaining accurate documentation, and ensuring a smooth onboarding experience for clients and families.
The ideal candidate is detail-oriented, customer-service driven, and capable of working in a fast-paced behavioral health environment while maintaining confidentiality and professionalism.
Essential Duties & Responsibilities
Serve as the primary contact for new client inquiries via phone, email, and walk-ins
Conduct initial intake screenings and gather demographic, insurance, and referral information
Schedule intake assessments and coordinate appointments with clinical staff
Verify insurance eligibility and obtain authorization when required
Maintain accurate client records within the electronic health record (EHR) system
Ensure all intake documentation, consents, and releases are completed and uploaded timely
Communicate with referral sources, hospitals, schools, community agencies, and families regarding intake status
Explain agency programs, services, policies, and procedures to clients and caregivers
Monitor intake pipelines and maintain referral tracking logs
Assist with collecting required documentation for admissions and program eligibility
Coordinate with clinical and administrative staff to ensure continuity of care
Follow HIPAA and confidentiality guidelines at all times
Assist with front office administrative duties as needed
Participate in staff meetings, trainings, and quality improvement activities
Must be willing to travel to clients home in order to fill form and get parent signatures. (Not in all cases)
Qualifications
Education & Experience
High school diploma or GED required
Associate’s or Bachelor’s degree in Healthcare Administration, Social Work, Psychology, Human Services, or related field preferred
Minimum of 1–2 years of administrative, intake, healthcare, or behavioral health experience preferred
Experience working with Medicaid, managed care organizations, or behavioral health services preferred
Knowledge, Skills & Abilities
Strong communication and interpersonal skills
Excellent organizational and multitasking abilities
Ability to handle sensitive information with discretion and professionalism
Knowledge of medical terminology and behavioral health services preferred
Proficient in Microsoft Office, Google Workspace, and EHR systems
Ability to work independently and as part of a team
Strong problem-solving and customer service skills
Physical Requirements
