Assist in locating patients in their homes to assure proper completion of the government application, obtain missing documentation, or to verify that third party insurance exists for the medical treatment provided. Responsibilities: o Conduct field visits to various locations including homes, businesses, etc. o Prepare all necessary applications and forms needed for field services prior to leaving the office o Prepare route of all field visits based on location prior to leaving the office o Transport patients to various locations as needed. o Act as courier between office and offsite staff, and to governmental facilities. o Cover an EA case load Requirements: o General knowledge of State and Federal Medical assistance programs. (AHCCCS, Medicaid, Medi-cal). o Must be familiar with governmental programs (AHCCCS, Medi-cal, Medicaid, etc.). o Exceptional Customer Service Skills o Great Computer Skills o Bilingual (Spanish & English) o Must provide immunization records, and tested annually for TB o Must have compassion o Annual Flu shots and pre-employment drug test o Reliable transportation Rate of pay: $13.00 to 15.00 per hour, based on experience Employee Type - Part-Time -24 hours per week
