Job Description
The Integrated Health and Outreach Program (IHOP) is a comprehensive service program focused on screening, testing, treatment, and referral for HIV, Hepatitis C, sexually transmitted infections (STIs), and substance misuse disorders. The Integrated Health and Outreach (IHO) Case Manager position serves as a member of a multi-disciplinary patient care team. They will be responsible for providing case management services that support the treatment goals of each patient’s care plan. The IHO Case Manager represents the company professionally and positively to enhance and promote the core values and mission of NATIVE HEALTH, always exercising the utmost discretion, diplomacy and tact interactions with patients, staff members, external partners, and public.
KNOWLEDGE, SKILLS AND ABILITIES:
- Knowledge of HIV, STIs, behavioral health case management principles, SDOH, and community resource coordination.
- Knowledge of case management practices.
- Two years of related case management experience is required. Experience working with underserved and diverse populations is preferred.
- Knowledge of culturally competent care models and trauma-informed approaches.
- Positive attitude and the ability to advocate for the patient’s needs.
- Excellent written and verbal communication skills and ability to establish rapport.
- Detailed and accurate documentation ability
- Ability to navigate multiple service systems and coordinate complex care across agencies.
- Proficiency in EHR systems and Microsoft Office; strong organizational and documentation skills.
- Professional integrity, sound judgment, and respect for client confidentiality.
- Ability to work collaboratively in interdisciplinary settings while maintaining clear boundaries.
- Familiarity with local, state, and federal behavioral health and social service regulations.
- Strong understanding of behavioral health terminology, diagnoses, and recovery models.
- Commitment to continuous learning, cultural humility, and inclusive practices.
- Participate in ongoing training, supervision, and quality improvement initiatives.
