• $72,000 - $80,000+ Depending on Experience

Requirements:

• Bachelor's or master’s degree in Social Work
• Licensed in the State of New Jersey
• Must Possess a Valid Driver’s License in The State of New Jersey, Current Automobile Insurance, And Reliable Transportation.

Responsibilities:

• Providing Psychosocial Assessments & Support to patients & Their Families in Patient’s Homes
• Discharge Planning
• Caseload Will Vary Depending on The Position, About 3 – 5 patients a day on average.
• Collaborate as Part of an Interdisciplinary Team
