Our client is offering the following benefits:
- Annual Salary: $53,000-$66,000 (Based on Experience)
- $10,000 Sign on Bonus

Responsibilities:
- Assesses Each Patient/Family Admitted For Psychosocial Issues Impacting on The Care-Giving System, Including an Assessment of The Environmental, Age Specific, Spiritual And Financial Impact of Loss on Patients, Families, Caregivers, And Clients.
- Formulates Individualized Plans of Care For Patients Based on Assessed Needs, Strengths, Limitations And Goals. Includes Patient And Caregivers in The Care Planning Process. Ensures That Patient’s Desires Are Taken Into Account When The Treatment Plan is Formulated. Regularly Reassesses For Changing Needs.
- Participates Actively in IDT Meetings, Functioning as an Advocate For The Patient And Family. Communicates Initial/Ongoing Plan of Care Appropriately to Multidisciplinary Treatment Team.
- Collaborates With Providers And Treatment Team to Identify And Evaluate Patient Outcomes And to Evaluate Variances in Expected Behavior And Appropriateness of Care.

- Licensed Graduate Social Worker (LGSW) in The State of Maryland
- Monday-Friday 8AM-4:30PM
- No Nights
- No Weekends
- Knowledge of EMR
