Requirements:

• Bachelor’s Degree in Social Work or Mental Health Counseling
• EMR Knowledge
• New Grads Welcome

Responsibilities:

• 8am-5pm M-F – Community Based Position
• 1 Late Evening a Week 11am-7pm
• Possibly 1 Saturday Month
• Provides Individual Support, Problem Solving, Training, & Supervision to Help Obtain The Necessities of Daily Living Including Medical And Dental Care, Legal Advocacy Services, Financial Support, and Transportation.
• Treatment Plans
• Case Managers Will Also Provide Education, Support, And Consultation to Clients.
