• Salary: $31.50/hr. to $44/hr

Job Responsibilities

Coordinates total patient care by conducting comprehensive health and psychosocial evaluation, monitoring the client's condition, promoting sound preventive practices, coordinating services and educational and training activities.

Submits clinical notes no less often than weekly; submits progress notes and other clinical record forms outlining the services rendered as indicated.

• Schedules; Sunday-Thursday OR Tuesday-Saturday

Job Requirements

• Registered Nurse in the State of Florida-Required Upon Hire
• 1 Year of Acute Care Experience will be considered
• 2 years of recent Home Health Experience-Preferred
• OASIS-D Experience – Preferred
