Enrollment Registered Nurse (RN)

The Position: We are looking for a full-time Enrollment/ Intake Registered Nurse (RN) to provide care to patient’s via their individual care plans. The client offers the following compensation package:
- $62,400- $80,000 Salary Based on Experience

Responsibilities:
- Monday- Friday Hybrid Schedule
- 8:00am- 4:00pm
- No Nights
- No Weekends
- Frequent Local Travel
- Responsible For Helping Patients Set Up Services
- Conduct Comprehensive Needs/ Medical Assessment
- This Position Requires Very Minimal Direct/ Bedside Patient Care
- Evaluate Patient's Functional Status, Caregiver Status, Current and Home Safety Status
- Present Assessment and Discuss with Interdisciplinary Team to Determine Needs and Services
- See Patient in Their Home, Complete Admin in Your Home
- No On Call Responsibilities
Requirements:
- MA RN Licensed
- New Grads Welcome
- CPR Certified
- Covid Vaccination Required
- MDS Assessment Experience Required ( Minimum Data Set)
- Must Have Experience Working with the Geriatric Population
- Build Strong Relationships With Participants
- Must Have Great Communication Skills
