• $31.25 to $36.54 Hourly, Based On Experience

Job Responsibilities

• Coordinates total patient care by conducting comprehensive health and psychosocial evaluation, monitoring the client's condition, promoting sound preventive practices, coordinating services and educational and training activities.
• Maintain a Case Load of about 25 patients, seeing about 7 patients per day on average.
• On Call as a Back-Up: The Triage RN and the Triage LVN will take most of the On-Call.
• Discusses the need for the involvement of other members of the health team such as the home health aide, the physical therapist, the speech therapist, the occupational therapist, and the medical social worker, etc., with the appropriate supervisor.

Job Requirements

• Registered Nurse in the State of Texas-Required upon hire
• 2 years of recent Home Health Experience-Preferred
• Kinser EMR Experience-Preferred
• Will Consider Home Health, Hospice, Home Care or Acute Care Experience
• OASIS-D Experience-Required
