Registered Nurse RN - Home Health

We are seeking a Registered Nurse for a full-time and 100% Home Health focused opportunity in Littleton or Lakewood, CO!

Compensation Package:

• $50 to $57 Per Regular Visit 
• $75 to $100 Per Non Medicare Visit 
• $85 to $110 Per Medicare Visit
• $65 to $70 Per Evaluation Visit

Requirements:

• Clear and Active RN License in Colorado
• At Least 6 Month of Home Health Experience
• Current and Active Driver’s License
• Experience with OASIS

Responsibilities:

The home health Registered Nurse (RN) aims to restore patients to their highest level of independence and well-being in the community, fostering self-management of diseases and identified processes. When full independence isn't feasible, the goal shifts to preparing patients and caregivers for ongoing care within their homes or other community health systems.

• Monday through Friday 8 AM to 5 PM
• 1 Saturday and Sunday Every 4 Weeks for Additional Compensation OR 2 Days Off
• Working with Geriatric Patients 
• Using Matrix Care EMR and OASIS Documentation
• Seeing 5 Geriatric Patients Per Day in Their Home - Routes Are Coordinated Depending on Your Home Location
• 1 to 2 Weeks of Onboarding/Orientation
