    Hospice Licensed Clinical Social Worker LCSW

    We are seeking a Hospice Licensed Clinical Social Worker who will work closely with the Director of Clinical Services and Hospice team for a full-time opportunity in Portland, Oregon!

    The Position: The Licensed Clinical Social Worker is responsible for the implementation of standards of care for medical social work services and for adherence to all applicable federal, state, local, and company policies & procedures which include providing care based upon the Social Work Practice Act. The Licensed Clinical Social Worker will serve as a member of an interdisciplinary group (IDG) and will collaborate with the team and the patient’s attending physician to develop an individualized written plan of care.

    Compensation Package:

    • $$40 to $48 Per Hour (Based on Experience)

    Requirements:

    • Master’s or Doctoral degree from a school of social work
    • Must hold a clear and active Oregon license as an LCSW
    • Minimum of one year's social work experience in a health care setting.
    • Experience in hospice and/or home health preferred
    • Demonstrates good verbal and written communication, and organization skills.
    • Ability to learn and utilize electronic medical record systems effectively and efficiently.
    • Maintains current BLS/CPR certification.
    • Active driver's license with an automobile that is insured

    Responsibilities:

    • Monday through Friday 8 AM to 4:30 PM
    • Assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s).
    • Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the registered nurse.
    • Carries out social evaluations and plans interventions based on findings.
    • Provides patient, caregiver, and family counseling.
    • Administer services that are ordered by the physician as indicated in the plan of care.
    • Communicates with the physician who is responsible for the patient's plan of care and other health care practitioners (as appropriate).
    • Assists physicians and other team members in understanding significant social and emotional factors related to health problems.
    • Prepares clinical notes on all patients referred to social work.
    • Provides information and referral services for Organization patients and families/caregivers regarding practical and environmental needs.
