Licensed Practical Nurse - Home Health

Compensation & Benefits Package:

• $57,000 - $67,500 (Pay Per Visit Model $55-$60/Visit)

Job Responsibilities:

• 5 to 6 Patients Per Day
• Practice safe delivery of physician ordered care to patients in home environment
• Assists with RN in carrying out the plan of care
• Medication management and administration
• Patient assessment while maintaining a holistic approach
• Provide accurate and concise documentation
• No On–Call
• EMR: Kinnser
• Flexible Schedule
• Co-ordinated routes to Limit Travel
  

Job Requirements

• Licensed Practical Nurse in the State of Illinois-Required Upon Hire
• 1 Year Home Health Experience - Preferred
• CPR for healthcare providers
• Ability to provide direct patient care
• High sense of urgency in meeting patient physical, emotional, and educational needs
• Active and Valid Driver’s License and Car Insurance

OR

• Recent High Acuity Experience (ER, ICU, PCU, OR, PACU, Med Surg, ETC.)
• OASIS-D Experience Preferred
