    Schedule: Full-time, anticipated 12-month base period with option period

    Position Overview

    CLFC Healthcare & Communications is seeking a highly experienced Internal Medicine Nurse Practitioner for an IHS medical center in Shiprock, New Mexico, the administrative capital of the Navajo Nation. This role provides direct patient care at the primary healthcare facility serving the Shiprock Service Unit, one of the largest and most active service units in the Navajo Area. The NP delivers comprehensive internal medicine services to a diverse patient population including adults and elders with complex chronic disease burdens. Shiprock sits at the heart of the Navajo Nation, surrounded by the iconic Shiprock pinnacle, the Chuska Mountains, and a community that values healthcare providers who bring both clinical excellence and cultural respect.

    Key Responsibilities

    • Provide comprehensive internal medicine services including evaluation, diagnosis, treatment planning, and ongoing management of adult patients with acute and chronic conditions
    • Manage complex chronic disease including Type 2 diabetes, hypertension, chronic kidney disease, COPD, heart failure, and metabolic syndrome in a population with elevated prevalence rates
    • Order, interpret, and act on diagnostic studies including laboratory panels, imaging, EKGs, and point-of-care testing
    • Prescribe medications, manage polypharmacy in elderly patients, and coordinate specialty referrals
    • Perform routine health maintenance examinations, preventive screenings, and immunization administration
    • Collaborate with physicians, pharmacists, behavioral health providers, and Community Health Representatives as part of an integrated care team
    • Document all patient encounters accurately and timely in the electronic health record system (RPMS/EHR)
    • Participate in quality improvement, chronic disease management programs, and population health initiatives

    Required Qualifications (75th Percentile Standard)

    • Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program
    • National certification as a Family Nurse Practitioner (FNP-BC) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP-BC) through ANCC or AANP
    • Minimum 5 years of clinical nurse practitioner experience in internal medicine, primary care, or family practice
    • Active, unrestricted APRN license in New Mexico (or eligible for NM licensure within 60 days)
    • Current DEA registration (or eligible)
    • Current BLS certification; ACLS preferred
    • Demonstrated experience managing complex chronic disease in medically underserved populations
    • Ability to pass a federal background investigation
    • U.S. citizenship or permanent resident status

    Preferred Qualifications

    • Prior experience working in Indian Health Service, tribal health, or urban Indian health facilities
    • Experience practicing in rural or frontier healthcare settings
    • Cultural competency training or demonstrated experience providing healthcare to Native American communities
    • Experience with IHS RPMS electronic health record system
    • DNP degree preferred
    • Experience with diabetes education and management programs
    • Familiarity with IHS credentialing and privileging processes
    • Experience with telemedicine/telehealth delivery models
