Job Summary
We are seeking an experienced General Practitioner to join our team! As a General Practitioner, you will be managing patients only is skilled nursing settings. You will be assessing their needs, ordering or performing tests, interpreting results, and offering diagnoses, prescriptions, and referrals, as needed. The ideal candidate must be self motivated and willing to contribute diligently to the growing practice.
Responsibilities – Skilled Nursing Facility Physician
Provide comprehensive medical care to residents in a skilled nursing facility, including assessment, diagnosis, and management of acute and chronic conditions.
Conduct regular rounding and timely evaluation of patients based on clinical needs, regulatory requirements, and facility policies.
Order and interpret diagnostic tests, labs, and imaging as clinically appropriate; communicate results and treatment plans clearly.
Prescribe and manage medications, including controlled substances, in accordance with federal and state regulations and evidence-based practice standards.
Maintain accurate, timely, and compliant documentation in accordance with CMS, state regulations, and facility EMR requirements.
Collaborate with nursing, rehabilitation, social work, pharmacy, hospice, and administrative teams to optimize resident outcomes and reduce avoidable rehospitalizations.
Provide timely response to nursing inquiries, change-in-condition evaluations, and urgent resident needs.
Support advance care planning, palliative care, and end-of-life discussions, incorporating resident and family preferences into care plans.
Monitor and participate in facility quality assurance and performance improvement (QAPI) activities, regulatory readiness, and infection control initiatives.
Maintain required continuing medical education (CME) and stay current with clinical guidelines, CMS updates, and best practices in geriatric and long-term care medicine.
Qualifications
All licensing and certification required to perform medicine in PA
Graduation from an accredited Medical School
Strong attention to detail
The ability to provide exceptional care to each and every patient
