    • ·Must be Licensed Clinical Social Worker by the GA Composite Board for Social Workers.
    • Ability to interpret the social, emotional, and behavioral problems as related to the medical condition and develop an appropriate treatment plan.
    • Demonstrated competence in reacting to and handling emergencies.
    • Able to work collaboratively with multiple health professionals in a busy and complex environment using tact, diplomacy, and discipline.
    • Strong charting/documentation skills.
    • Ability to effectively communicate with people at all levels and from various backgrounds.
    • Must be able to speak, read, write, and understand the primary language(s) used in the workplace.
    • Good judgement with the ability to make timely and sound decisions
    • Strong organizational, problem-solving, and analytical skills
    • Ability to manage priorities and workflow
    • Versatility, flexibility, and a willingness to work within constantly changing priorities with enthusiasm.

    POSITION SUMMARY:

    The J.C. Lewis Primary Health Care Center is seeking to add a LICENSED CLINICAL SOCIAL WORKER. The LCSWsupports the team by providing medical services, within the scope of practice, at the request of the Health Care Provider.

    PRIMARY RESPONSIBILITIES:

    Provide services to children, adolescents and adults with mental illness, behavioral health and/or substance abuse disorders

    • Provide therapy, individual and group counseling and behavioral interventions including conducting assessments, developing/updating treatment plans and various tasks pertinent to case management

    • · Completes psychosocial assessments on patients as referred.

    • Develops and executes psychosocial treatment plans and associated documentation.

    • Identifies and addresses barriers to care and coordinates effective resolution.

    • Collects, analyzes, and interprets data for trending, reference, problem identification, and problem solving.

    • Provides crisis intervention counseling as needed.

    • Prioritizes patient referrals and needs.

    • Advises, counsels, and when appropriate instructs family in patient’s social needs.

    • Assists patient and when appropriate patient’s support system in problem identification, analysis, suggestions for interventions, and follow-up plans.

    • Reports suspicions of child and/or adult abuse to the appropriate authorities according to statutes.

    • Maintains a patient caseload; reviews cases on an on-going basis to assume maintenance of accurate information, benefits, and appropriateness of the plan of treatment.

    • Exhibits a high degree of courtesy, tact, and poise when interacting with patients, families, and other healthcare professionals.

    • Adjusts to fluctuating peaks in patient flow, acuity, and other operational demands while maintaining quality.

    OTHER DUTIES AND RESPONSIBILITIES

    • Performs clerical duties and completes reports as assigned
    • Maintain certification/licensure, to include the procurement of appropriate CMEs
    • Participate in agency meetings, in-service trainings and workshops as deemed appropriate
    • All other duties as assigned
