    Title:                Housing Services Coordinator

    Manager:       Sr. Program Manager

    Status:           Full Time, Hourly

    Location:        North Orange County, CA

    PRIMARY OBJECTIVE OF POSITION:

    The Housing Services Coordinator is responsible for providing housing navigation services, life skills services, and resource referral services to participants in Jamboree Communities. Housing services are provided to assist the participant in increasing their ability to manage the domains of independent living. Linkage and referrals are provided to ensure the participants are receiving all necessary services.

     

    MAJOR AREAS OF ACCOUNTABILITY:

    · Maintain a caseload of current and former residents.

    · Provide case management support, goal planning, and housing navigation throughout their participation in the program and 12 months of supportive services post program exit.

    · Outreach to tenants through regular home visits and telephone contacts and providing a “listening ear” one-on-one to all tenants.

    · Complete Intake of Service Needs and participant service plans and update as necessary.

    · Provide case management at least one time per week to each individual and additional case management as needed.

    · Assist tenants in integrating and engaging in developing community connections.

    · Monitor for warning signs of de-compensation and crisis and responding accordingly.

    · Facilitate groups pertaining to living skills as needed.

    · Support tenants in connecting to education, employment, financial literacy opportunities.

    · Provide supportive services and case planning to address issues and barriers that negatively impact progress toward goals, permanent housing opportunities or retention.

    · Provide support to participants experiencing homelessness in identifying permanent housing solutions through landlord outreach, development of a housing stability plan, and assist in activities that would prepare resident for stable housing.

    · Administrative duties will consist of data entry of all tenant information in HMIS and agency systems, creating both an electronic and hard file for each tenant in program, and maintain tenant roster of all tenant’s post program.

    · Complete all necessary documentation including intake assessment paperwork, living skills inventory, treatment plans, housing stability plan, strengths assessment, outcome measures, referral forms and progress notes for each client.

    · Collaborate with community partners and homeless services providers to coordinate and provide linkage to necessary appointments and services.

    · Attend Case Management and care coordination meetings with external parties as needed.

    · Provide transportation assistance as needed

    · Other duties as assigned.

     

    LEVEL OF EDUCATION, EXPERIENCE AND SKILL SET:

    · Bachelor’s degree preferred; or equivalent experience serving homeless individuals

    · Over 21 years of age.

    · Experience working with individuals experiencing chronic homelessness and mental health challenges

    · Familiarity with the Housing First, Harm Reduction and/or Recovery Oriented service models

    · Knowledge of local Coordinated Entry System (CES).

    · Ability to interact in a culturally diverse community

    · Ability to work effectively in an interdisciplinary team setting

    · Valid California driver’s license and availability of an insured vehicle.

    · Job includes some evening and weekend hours and travel throughout California.

    PHYSICAL DEMANDS:

    The Housing Services Coordinator is required to talk and hear.  The employee is often required to sit and use their hands and fingers to handle or feel.  The employee is required to stand, walk, reach with arms and hands, climb, balance, twist, lean, move from one location to another and to stoop, kneel, crouch or crawl.  Vision abilities required by this job include close and far vision.  Ability to operate a computer keyboard and lift files and reports.  Exposure to glare from a computer.

    WORK ENVIRONMENT:

    Working with the homeless population whether on the streets, in shelters or other places of habitation or services. Exposure to bed bugs or other infestations, unpleasant smells or odors, unclean individuals or homes due to homelessness or mental health symptoms or poverty. Some work will be in a recreational room or common area. The noise level is usually moderate but will vary depending on the activity participating in.
