    Position Summary

    The residents Case Manager is a mid-level social services and advocacy position responsible for proactively working with residents of Jan Peek House, community service providers, including educational and employment resources, DSS, housing programs and landlords to assist residents with obtaining stable housing and services leading to independence and long-term stability. This position requires flexibility of approach and willingness to utilize various service modalities to assist residents with achieving their goals.

    The ideal candidate will bring a readiness to learn and provide support to empower residential clients to obtain and maintain stable housing and long-term independence.

    Key Responsibilities

    Conduct timely intake and assessment of new residents

    Meet with residents to develop, implement, and review individualized service plans

    Provide support services related to independent living skills including budgeting, connection to community resources, and access to support, employment and health services.

    Provide advocacy for residents for services and benefits

    Assist residents with securing stable housing

    Ensure Emergency Service (ES) clients receive timely, comprehensive assessment of immediate needs, risks, and service requirements

    Administrative & Team Support

    Maintain files “audit ready”

    Work across programs when needed

    Participate in staff meetings, conferences, and organizational events

    Provide data and reports

    Qualifications

    Required

    Bachelor’s degree or equivalent experience in social justice studies, social services, or a related field

    Minimum 2 years’ experience in a related nonprofit field

    Strong written and verbal communication skills

    Excellent organizational skills and attention to detail

    Ability to manage multiple tasks and meet deadlines

    Proficiency with Microsoft Office and/or Google Workspace

    Proficiency with client database systems

    Valid driver’s license and access to a vehicle for occasional local travel

    Demonstrated commitment to equity, dignity, and the mission of addressing housing and food insecurity

    Preferred

    Spanish/English bilingual

    Knowledge of issues related to homelessness, disability, and trauma
