Compassionate Nursing Caregiver Needed Job Overview:

We are seeking a caring and dedicated nurse to provide high-quality comprehensive nursing care to individuals in need. As part of our team, you will coordinate the delivery of cost-effective multidisciplinary healthcare services to ensure the well-being of our patients. This role requires working under moderate supervision.

Responsibilities:

• Assess the physical, social, and psychological needs of patients through interviews, observations, and examinations to develop personalized care plans.
• Understand and implement reimbursement protocols, consulting with management on complex cases.
• Coordinate care with a multidisciplinary team, including referrals to other agencies and ordering necessary supplies and services.
• Accurately document services provided in compliance with regulations and organization policies.
• Assist in the orientation of staff and mentor students as needed.
• Provide nursing care following organization's standards, which may require physical tasks such as bending, stooping, and sitting.
• Transport medical equipment, weighing approximately 25-30 lbs., to and from patient homes or care facilities.
• Travel to various environments to deliver care, respecting the unique needs of each location and patient.
• Maintain productivity to meet organizational goals and participate in professional development opportunities.
• Perform other related duties and special projects as assigned.

Qualifications:

• License and current registration to practice as a registered professional nurse in New York State required.
• Valid driver's license or NYS Non-Driver photo ID card, may be required based on operational/regional needs.
• Degree in Nursing from an approved program required.
• Minimum of one year of experience in a medical/surgical environment required.
