Job Description We are looking for individuals who are passionate about patient-centered care and want to make a difference in people's lives. In this role, you will be supporting and participating in the Patient-centered Medical Home (PCMH) concepts of care coordination and team-based care. Your responsibilities will include: - Following established workflows for utilization and self-management support - Participating in PCMH activities such as hospital discharge follow-up, care planning, coordination of services, and communication between care providers - Collaborating with patients, their families/caregivers, and medical/health providers to establish mutual goal-setting and promote self-management - Providing outreach, care management, and education for disease self-management based on patient-centered goals - Identifying and addressing barriers to improved outcomes for patients - Monitoring and evaluating services and community-based resources to meet individual health needs - Conducting assessments, developing nursing treatment plans, setting goals for behavioral modifications, and collaborating with other care providers - Assisting in implementing evidence-based practices and care plans for target populations - Collecting data on quality metrics and participating in process/quality improvement initiatives Experience Required We are looking for candidates with at least 5 years of clinical experience, preferably in primary care, population, or case management. Education Requirements RN (BSN) preferred, but we consider all qualified applicants. License/Certification Requirements Applicants must have a current NJ RN License.
