Weekend Care Coordinator

Responsibilities:

The Weekend Care Coordinator will be responsible for coordinating patient care, assessing the needs of patients and their families, and implementing the interdisciplinary care plan. They will ensure that care is delivered in compliance with federal and state regulations.

Requirements:

• Graduation from an approved nursing education program
• Valid nursing license in the state of practice
• At least one year of experience as a licensed registered nurse
• Basic technology skills, including the use of mobile devices

Preferred qualifications:

• Experience in Medicare home health or hospice

Additional Requirements:

• Valid state driver's license
• Automobile liability insurance as required by law
• Dependable transportation in good working condition
• Ability to drive safely in all types of weather conditions
• CPR certification for healthcare provider
