Job Title: RN Home Visitor Schedule: Part-time (20 hours/week), Monday - Friday, variable day shift Summary: As an RN Home Visitor, you will play a multifaceted role that demonstrates professionalism and promotes excellence in nursing. Your responsibilities will include delivering case management and support services to a designated caseload of patients, maintaining high standards in clinical practices, professional development, teamwork, customer relations, and research/performance improvement. You will follow the guidelines outlined in the Nurse-Family Partnership Program. Responsibilities: 1. Conduct home visits with pregnant/parenting women and their children under the program research protocols. 2. Collaborate with community agencies as needed, following guidelines. 3. Ensure compliance with the Ohio Nurse Practice Act and other applicable regulatory standards. 4. Adhere to policies, procedures, and protocols of the Human Resources Department, Children's Home Care Group, contracting entities, and the Nurse-Family Partnership research project. 5. Comply with all applicable regulatory standards, policies, procedures, and protocols of the Human Resources Department, Children's Home Care Group, and funding sources. 6. Demonstrate knowledge and skills to provide comprehensive care for patients of all ages, addressing their physical, psychological, social, educational, and safety needs. 7. Perform other duties as required. Requirements: - Good communication skills and ability to work with adolescents. - Possession of a valid driver's license and an insured, safe automobile. - Education: BSN required. - Certification: Registered Nurse licensure and BLS certification. - Relevant experience: At least two years of recent pediatrics or related experience with children birth through 5 years. Additional experience in adolescent psychology, OB, home care, and community nursing preferred.
