    Position Details:

    • Full-Time: 40 hours per week
    • Shift: 1st Shift - 8am to 4:30pm, Monday to Friday
    • Work Arrangement: Remote with onsite requirements as needed
    • Location Requirement: Applicant must reside in Ohio or Pennsylvania upon offer

    Summary:

    This position is responsible for coordinating care for patients and families with behavioral health needs within Akron Children's Hospital Pediatric Clinics (ACHP). The Care Coordinator will work with patients, families, and the treatment team to assess needs, coordinate care, and monitor progress. Services will be provided in-person and remotely to support multiple ACHPs.

    Responsibilities:

    1. Identify and triage patients at acute risk of suicide and arrange transportation to appropriate crisis services in collaboration with the Primary Care Provider.
    2. Participate in case conferences to develop treatment plans with the entire team, including primary care providers and psychiatry staff.
    3. Monitor symptoms using evidence-based tools and maintain a registry to track treatment progress. Assist in coordinating further care for patients not showing expected improvement.
    4. Assist with navigation between different levels of care within Behavioral Health.
    5. Help families schedule behavioral health appointments, identify barriers to engagement in treatment, and provide basic motivational interviewing interventions to address barriers.
    6. Educate patients and families about behavioral health conditions and support medication treatment, as necessary.
    7. Assess social determinants of health and make referrals as needed, coordinating with school health and community organizations when necessary.
    8. Document all activities in the medical record.
    9. Effectively work with patients and families from diverse backgrounds.
    10. Perform other assigned duties.

    Other Information:

    Technical Expertise:

    • Experience working with multidisciplinary teams.
    • Proficiency in Electronic Health Record computerized documentation is preferred.
    • Experience in healthcare is preferred.
    • Excellent communication skills, both verbal and written.
    • Strong organizational skills with the ability to multitask and problem-solve.
    • Proficiency in MS Office or similar software is required.

    Education and Experience:

    • Education: High School diploma or equivalent is required.
    • Licensure: Not applicable
    • Certification: Not applicable
    • Years of relevant experience: 2-4 years preferred. A bachelor’s degree in a related field may substitute for experience.
    • Years of supervisory experience: None
