Candidates must live in Oklahoma

Position Purpose:

• Assess the needs of our members and provide interventions according to program guidelines.
• Coordinate care with community resources and member advocates.
• Educate providers, community resources, and members about program components and available support services.
• Develop and modify care plans in collaboration with the member, their family, and managing physician.
• Create educational materials and resources for specific care plans.

Education/Experience:

• Bachelor's degree in Social Work, Nursing, Health, Behavioral Science, or equivalent experience.
• 2+ years of experience in managing and integrating social/community needs of members.
• Experience in a managed care environment is preferred.

License/Certification:

• Valid driver's license (except for Indiana).
• Preferred: RN (Registered Nurse), LCSW (Licensed Certified Social Worker), LPN (Licensed Practical Nurse), Occupational Therapy (OT), Physical Therapy (PT), or LVN (Licensed Vocational Nurse)
