Candidates must live in Oklahoma
Position Purpose:
- Assess the needs of our members and provide interventions according to program guidelines.
- Coordinate care with community resources and member advocates.
- Educate providers, community resources, and members about program components and available support services.
- Develop and modify care plans in collaboration with the member, their family, and managing physician.
- Create educational materials and resources for specific care plans.
Education/Experience:
- Bachelor's degree in Social Work, Nursing, Health, Behavioral Science, or equivalent experience.
- 2+ years of experience in managing and integrating social/community needs of members.
- Experience in a managed care environment is preferred.
License/Certification:
- Valid driver's license (except for Indiana).
- Preferred: RN (Registered Nurse), LCSW (Licensed Certified Social Worker), LPN (Licensed Practical Nurse), Occupational Therapy (OT), Physical Therapy (PT), or LVN (Licensed Vocational Nurse)
