Short Description We are looking for a caring individual to serve as a point of contact for newly diagnosed HIV patients who are enrolling in our medical care services. In this role, you will also help reconnect HIV patients who may have stopped receiving care. Additionally, you will provide partner services to both new and existing HIV patients, working closely with our medical care team to offer counseling and support throughout their treatment journey. It is important that you employ social networking strategies to encourage HIV testing among new and existing patients, and provide prevention counseling and strategies to patients and their families. This position plays a key role in our multi-disciplinary care approach. Experience Required You should have at least 1 year of experience providing HIV prevention or care services to people living with HIV. Experience in counseling HIV-positive individuals on accessing available services and knowledge of community resources is essential. Education Requirements A BSN (Bachelor of Science in Nursing) is required for this position. You must also hold a current NJ RN (Registered Nurse) license. License/Certification Requirements You must possess a current NJ-RN license and have a BSN qualification. Special Requirements This is a full-time position, with 40 hours per week. A current NJ-RN license and a BSN qualification are required. Training in clinical navigation provided by the Division of HIV, STD, and TB Services (DHSTS).
