Short Description
The Project Manager (PM) will work on quality and performance measurement for (Center for Medicare and Medicaid Services (CMS) value-based programs such as the Medicare Shared Savings Programs (MSSP), Oncology Care Model (OCM), End Stage Renal Disease (ESRD), Medicaid Delivery System Reform Incentive Payment program (DSRIP) and Patient Centered Medical Home (PCMH). The role will require a strong analytical background, along with quality improvement methodology and implementation experience.
The PM is responsible for managing the project activities to improve health care quality and cost-effectiveness for Medicare, Medicaid, charity care, and dual eligible populations as outlined by CMS . This individual will convene and lead interdisciplinary teams of health care professionals and community-based organizations to develop and implement strategies to decrease avoidable utilization of hospital-based services, increase use of community-based services and support patients in self-care. The individual will coordinate plans for CMS projects across the continuum of care and manage additional projects such as PCMH Recognition that support the Health system’s ability to achieve the triple aim.
To accomplish successful implementation of CMS programs, PCMH and other projects, the PM will work closely with Population Intelligence and Innovative Delivery team members, Performance Improvement (Quality) and Process Improvement Office(PIO) to identify and facilitate improvements within the care continuum, transitions of care and service line settings.
