The Registered Nurse (RN) Case Manager role involves coordinating client care and being the main point of contact for all disciplines involved in patient care. The RN Case Manager consults with physicians and informs the office about patient care. Other responsibilities include accurate OASIS collection, keeping the medication profile updated, timely reporting of lab values to the physician, and attending weekly case conferences and monthly case manager meetings.
This part-time salaried position requires working 24 hours per week over 4 days and is eligible for our 60% paid days off program.
Registered Nurses (RNs) must be currently licensed in the state of employment and preferably have at least one year of clinical experience. They should also demonstrate knowledge and skill in current nursing practices and possess a valid state driver’s license and automobile liability insurance.
